2025 Foundation Representative Survey

Dear leaders,

Thank you for your ongoing dedication to our foundation partners.

To gauge how these relationships are going, we ask that you please complete the following short survey. The survey will remain open until  Wednesday, Dec. 17 at 4 p.m.

The phrase "health system" refers to the Provincial Health Agencies and the Provincial Health Corporations including Acute Care Alberta, Assisted Living Alberta, Primary Care Alberta, Recovery Alberta, AHS, Health Shared Services, Cancer Care Alberta, Emergency Health Services, and Give Life Alberta.
