Palliative care and end-of-life care are closely related but different. Palliative care is a comprehensive, patient-centered approach that aims to improve the quality of life for someone with a serious illness by relieving symptoms and supporting their physical, emotional, psychological, social, and spiritual needs. In contrast, end-of-life care focuses on individuals in the final stages of life, with the primary goal of ensuring comfort, dignity, and support for the individual and their family during their last weeks or months.
At the Ponoka and Wetaskiwin Long Term Cares, we are committed to refining the quality of palliative care and end-of-life care that we provide.
We are inviting your feedback to help us understand what we are doing well and where we can grow. This survey will be used to guide improvements in care and communication.
This anonymous survey will take approximately 5-10 minutes to complete. Please do not include any personal information or names in your response.
*PPS - Palliative Performance Scale: a tool used to assess each person's functional status. We will be implementing it to help guide care planning, improve communication among care teams, and ensure appropriate supports are available at the right times for residents and their families.
