Patient and Family Centred Care Week held November 2-6, 2020, empowers individuals, teams, sites and zones to celebrate our partnerships with patients, clients, residents and their families.

What is Patient and Family Centred Care?

Patient and Family Centred Care encompasses four core concepts, including:

Respect and Dignity. We listen to and honour patient and family perspectives and choices. We include patient and family knowledge, strengths, values, beliefs, and cultural backgrounds into the planning and delivery of care.

Information Sharing. We communicate with patients and families to ensure they understand and receive timely, complete, and accurate information in order to effectively participate in care and decision making.

Participation. We build partnerships and encourage and support patients and families to participate in care and decision making at the level they choose.

Collaboration. We collaborate with patients and families in policy and program development, implementation, and evaluation.

Small Things Matter

We recognize that 2020 has been a challenging year, but it has also brought us together in ways we never expected.  

In celebration of Patient and Family Centred Care Week, let's pause and recognize each other – patients, families, and healthcare providers for the acts of kindness and resilience that have kept us going during these times.

  • Staying Connected

    Last March when we shifted to working virtually from home, my manager immediately put in place a daily team meeting for us to stay connected to each other. It was a time to talk about changes in our work, but also a time to share how we were managing and coping with the change and stress of COVID. I am so grateful for this daily meeting. It kept us connected, improved my mental health, and I felt very supported.
