Immunizations Required to work as a Healthcare Provider in Alberta

When you start work in Alberta, you will be expected to provide immunization records and/or immunity testing records for several communicable diseases. Please work to gather these records now.

All new employees are required to complete a Communicable Disease Assessment (CDA) form. For more information, see the Immunization Instruction Sheet.

Important: You will be required to complete and submit the form within 90 days of starting employment. However, if the unit you are assigned to work at is on “outbreak” status and you are unable to provide proof of vaccination or immunity, you will be sent home and remain off work WITHOUT PAY, until such time as you can provide proof.
