Each year, Advisory Council members participate in an evaluation to reflect on the past and help plan for the future.
You will find the following surveys posted below:
2020 Annual Council Survey: We need all members, both experienced and new to complete the annual survey. It is an important annual measure we use for reporting to the Board and senior leadership and for setting goals and targets for the coming year. In previous years, this survey was the catalyst for many of the changes you are seeing today as part of HAC 2.0. The purpose of this survey is to:
Hear from individual Council members
Identify strengths and areas of improvement
Collect ideas for 2021
If you are unable to complete the survey online, we can accommodate phone surveys. Please contact us at community.engagement@ahs.ca if that is a preferred option.
Council Events Survey: This survey is for all Councils who have recently implemented a community event, such as Psychological First Aid training or a community conversation, as part of the HAC 2.0 model. The purpose of this survey is to:
Complete a pulse check on Council events held to date
Assess the effectiveness of events
Assess the event planning process
Hear from Council members on what’s working well, what can be improved, and ideas for future Council events
Both surveys will be live from December 1 - 15. All responses are anonymous. Results will be summarized and shared back to your Council, and to applicable staff and leaders.

Many Health Advisory Councils have recently hosted a community event as part of the HAC 2.0 model. This survey is intended as a mid-point evaluation for members to provide feedback on their events. Those who have not yet had the opportunity to hold a community event will have a chance to provide feedback on their events at a future date.
